Welcome to Middlesex Titans Sports Social Club. This Junior Membership Form should be completed by the parent or legal guardian of any player under the age of 18 and must also be signed by the player.

Please complete all sections of this form and return it to the relevant age group manager or membership secretary.

We will also use this information to ensure that you are kept informed about events and information concerning Middlesex Titans Sports Social Club.

Junior Training Registration Windows:
Please note that registration for Junior Training is only open during the following periods:

  • Autumn: 1st October to 31st December
  • Winter: 1st February to 30th April
  • Summer: 1st May to 15th June

While it is not compulsory for these sections to be completed, it is important. Sport can and does play a major role in promoting the inclusion of all age groups in society. However inequalities have traditionally existed within sport, particularly in relation to gender, race and disability. Sport England is committed to promoting and developing sports equity, which is about fairness in sport, equality of access, recognising inequalities and taking steps to address them. By monitoring the profile of young people in sports clubs, national governing bodies of sport and Sport England can identify and issues relating to under-representation of different groups and can together develop strategies to ensure all young people have an opportunity in the future development and progress in sport.

Data Protection


The Club will use the information provided on this Membership Form (together with other information it obtains about the player) to administer his/her cricketing activity at the Club and in any activities in which he/she participates through the Club and to care for and supervise activities in which he/she is involved.



In some cases, this may require the Club to disclose the information to County Boards, Leagues and to the England and Wales Cricket Board. In the event of a medical issue or child protection issue arising, the Club may disclose certain information to doctors or other medical specialists and/or to police, children’s social care, the Courts and/or probation officers and, potentially to legal and other advisers involved in an investigation.





Declaration


As the person completing this form, you must ensure that each person whose information you include in this form knows what will happen to their information and how it may be disclosed.



  1. By returning this completed Junior Membership Form, I agree to my child / the child in my care taking part in the activities of Middlesex Titans Sports Social Club
  2. I confirm that I have legal responsibility for the child named in section 1 above, and that I am entitled to give this consent.
  3. I understand that I will be kept informed of activities at Middlesex Titans Sports & Social Club – for example details of times and transport etc.
  4. I understand that in the event of injury or illness all reasonable steps will be taken to contact me / the alternative contact, and to deal with that injury/illness appropriately.
  5. I confirm that to the best of my knowledge all information provided in this form is accurate and I will inform the club of any changes to this information in a timely manner.
  6. I confirm that I have received a copy of the club’s Code of Conduct for Members and Guests and the Junior Code of Conduct and my child agrees to abide by them.
  7. I agree that my child will be a playing member of the Club and that parents are Honorary Non-Playing Members, and agree to be bound by the constitution and rules of the Club. I understand that as a parent, I am eligible to sign as a Social Member paying the relevant membership cost.
  8. I understand that Club membership is by invitation and on acceptance and confirmation, MTSSC Junior Members will be governed by the rules and regulations of the Club & Committee. Membership can be withdrawn by the Club at any time for breach of the Club’s code of conduct, rules, regulations or terms.   9. I understand that membership will be provisional for an initial period of 3 months.
  10. I understand the details on this form will be used to provide club updates.
  11. I understand this form needs to be completed annually as per ECB and MCB guidelines.


Agreement

The section below is to be completed by the child applying for Junior Membership



  1. I agree to my personal information (which may be in electronic form) being held by the club for administrative and social purposes, in accordance with its Data Protection Policy.
  2. I understand that as part of the Middlesex Colts' Association's conditions, on acceptance of this membership, I will be registered with www.mtssc.play-cricket.com and other related websites.

    Your unique User ID & Password details will be despatched to you by the Membership Secretary.
  3. I agree to the terms & conditions stipulated on this form, confirm that I have read a copy of the MTSSC Code of Conduct for Junior Members and Guests and agree to abide by the Code’s provisions.
  4. I understand the details on this form will be used to provide club updates.
  5. I understand this form needs to be completed annually as per ECB and MCB guidelines.
